BENEFITS PROVIDED

DOCTOR’S BENEFITS

 A member must contribute 4 weekly contributions to receive 50% of benefits
A member must contribute 8 weekly contributions to receive 100% of benefits
The union must be in receipt of the monies before visitation cards will be issued.
 

FUNERAL BENEFITS

 
  1. Union allocates R7 000.00 for member.
  2. R5 000.00 for spouse.
  3. An amount of R8 000.00 is available to cover beneficiaries (children and biological parents only).
  4. Amounts claimed for beneficiaries, shall be limited to a minimum of R1 000.00 and a maximum of R2 000.00, per beneficiary.
  5. Money is paid into member’s bank account (E.F.T) on receipt of death certificate and ID’s of member and deceased person.
  6. The qualifying period is six (6) months for member and spouse and 12 months for all other beneficiaries. A funeral benefit claim must be lodged within six (6) months. Nine months cover is granted for the member only in the case of retrenchment.
 

BURSARY BENEFITS

 Must contribute for 1 year to qualify. (Financial assistance will be given for the following)

 

  1. Grade 8 – Grade 12
  2. Technikon
  3. University
  4. College
 

ANNUAL LIMITS FOR DOCTOR VISITS

 
  1. Member with (0) dependants – 8 Doctors visits
  2. Member plus (1) dependants –  8 Doctors visits
  3. Member plus (2) dependants – 10 Doctors visits
  4. Member plus (3) dependants – 12 Doctors visits
  5. Member plus (4) or more dependants – 15 Doctors visits
 

PANEL DOCTORS

  • You and your dependants must consult only the Panel doctors. If a member consults a private doctor, the member must pay the doctor and submit the receipt with his doctor’s visitation card, to the Union offices for re-imbursement. Receipt must be stamped by Doctor and Employer.
  • Member will be refunded at rates paid to panel doctor’s if member paid more than those rates
 

UNITED TO PROTECT NOT COMBINED TO INJURE

SPECTACLES

 
  • A member must contribute for 1 year in order to qualify and thereafter qualify every second (2) year. When claiming benefits in respect of spectacles, a member must first contact the Union office to verify if he/she is in benefit.
  • Member to present a receipt of payment and a detailed invoice to the Union when claiming.
  • Member is liable to pay the optician and send receipt for refund. No payments will be made to any optician.
  • R900.00 is allocated towards optical benefits for a member and his/her dependants.

 

 

DENTAL BENEFITS

 
  • When claiming benefits in respect of dentures or dental services, a member must present a receipt of payment to the union office, with the doctor’s stamp and work stamp.
  • Qualifying period 26 weeks contributions and thereafter, annually.
  • R1000.00 is allocated per family towards dental/dentures. This is a combined benefit.

 

 

CONFINEMENT GRANT

 
  • The union does not pay for pregnancy but a confinement is granted after 26 weeks of contributions.
  • 00 is allocated for the first claim and R250.00 for the second and third claim.
NB: CONFINEMENT WILL BE LIMITED TO 3 CLAIMS PER MEMBER.
 NB: Only members who pay funeral or illhealth qualify for confinement grant.
 

HOSPITAL ACCOUNT

 
  • When treated at provincial hospital, you will be assessed according to your income. You must therefore take your latest tax assessment form and proof of income to qualify for a rebate. Only then, must you submit your hospital account for payment to the Union office. The amount allocated towards Hospital Benefits is R3000.00, per member and dependants, per annum.
 
 

REGISTRATION OF DEPENDENTS

  • To register wife:   Submit marriage certificate and wife’s I.D
  • To register child/ children:   Submit copy/copies of children’s birth certificates
  • An affidavit and birth certificate is required if child’s surname is different from that of member.
  • If child is older than 18 years and still at school, then proof from the school is required along with the child’s Birth Certificate or copy of I.D. Document.
