DOCTOR\u2019S BENEFITS<\/span><\/strong><\/h4>\n
\u00a0<\/span>A member must contribute 4 weekly contributions to receive 50% of benefits<\/h5>\n
A member must contribute 8 weekly contributions to receive 100% of benefits<\/span><\/h5>\n
The union must be in receipt of the monies before visitation cards will be issued.<\/span><\/h5>\n
\u00a0<\/span><\/h6>\n

FUNERAL BENEFITS<\/span><\/strong><\/h4>\n
\u00a0<\/span><\/h6>\n

  1. \n
    Union allocates R7\u00a0000.00 for member.<\/span><\/h5>\n<\/li>\n
  2. \n
    R5\u00a0000.00 for spouse.<\/span><\/h5>\n<\/li>\n
  3. \n
    An amount of R8\u00a0000.00 is available to cover beneficiaries (children and biological parents only).<\/span><\/h5>\n<\/li>\n
  4. \n
    Amounts claimed for beneficiaries, shall be limited to a minimum of R1 000.00 and a maximum of R2\u00a0000.00, per beneficiary.<\/span><\/h5>\n<\/li>\n
  5. \n
    Money is paid into member\u2019s bank account (E.F.T) on receipt of death certificate and ID\u2019s of member and deceased person.<\/span><\/h5>\n<\/li>\n
  6. \n
    The qualifying period is six (6) months for member and spouse and 12 months for all other beneficiaries. A funeral benefit claim must be lodged within six (6) months. Nine months cover is granted for the member only in the case of retrenchment.<\/span><\/h5>\n<\/li>\n<\/ol>\n

        PANEL DOCTORS<\/span><\/strong><\/h4>\n

          You and your dependants must consult only the Panel doctors. If a member consults a private doctor, the member must pay the doctor and submit the receipt with his doctor\u2019s visitation card, to the Union offices for re-imbursement. Receipt must be stamped by Doctor and Employer.<\/span><\/h5>\n<\/li>\n

          Member will be refunded at rates paid to panel doctor\u2019s if member paid more than those rates <\/span><\/h5>\n<\/li>\n<\/ul>\n
